Also some funky looking card backs.
Do you get these from winning tournaments or something?

some of the cosmetic stuff is unlucked by leveling up. Some can be purchased in the store. Some are given as rewards for reaching specific ranks during a season. Some are part of the kickstarter rewards package. And I think some have been given out as tournament rewards.
The card backs in particular are rewards for reaching high rank during a season. If you reach rank 15+, you will be given a specific card back along with 3 pandora tickets. If you reach God, you will receive a different card back along with 5 pandora tickets.
